Early Alert Form

Early Alert is a team of faculty and staff who support referred students by linking them with appropriate resources.  The team does not discipline students, but rather helps students overcome barriers to success.  EarlyAlert assists with retention on a student-by-student basis, supporting the goal of the Retention Management Team, which works on student retention through planning and systems. 

The Information provided is confidential and will only be shared with those that need to know on a professional basis.  The Dean of Students will ensure appropriate follow up.  Please complete all information and submit this form to the Office of Student Development, 117 Lenfest Commons.  If you have any questions or comments please feel free to contact the Dean of Students at earlyalert@wilson.edu.
