Daily Health Check

The Wilson College Honor Principle calls on members of the campus community to “respect others and behave with the interest of the whole community in mind.” In light of the current COVID-19 pandemic, Wilson College has implemented guidelines to create the safest environment possible for everyone in its offices, classrooms, public spaces and residence halls. The success of these measures depends entirely on the commitment of all members of the Wilson College community to follow the guidelines.

As part of these guidelines, all members of the campus community are required to complete a daily health assessment before attending class and/or traveling to campus. A mobile app is under development for this purpose. Until that tool is available, you are asked to complete this brief questionnaire instead. This information will be kept confidential and reviewed daily to gauge the health of campus. Thank you in advance for your participation.

(This number will only be used to contact you to discuss your health assessment results, should it be necessary.)
